Documents Needed to File
You will need the following to file your tax return.
Valid Photo ID
Social Security number for you and your dependents included on return
Income documents( W2, 1099(G,K,R,MISC, NEC), Income Statements(Self Employed))
Completed Intake form (Click here to access)-This must be completed at least 48hrs before your scheduled appointment. Your time is valuable to me. To eliminate unnecessary time spent sitting, please have this form completed.
For Self Employed: Please have documentation showing income and expenses of the business for the year.
